School Absent Note Checklists

If your child is out from school for any reason, schools usually request that your child bring in an absent note from home (or from the doctor if they are out for a few days). We've created a sheet of 3 absent notes on a page, so you can cut out one, fill it in and send it with your child to school. Fill in your child's name, class information, which days they were out, reasons, and sign/date it. Also included is a space for teacher's name and student ID. Great for last minute notes! We hope this checklist helps you :).

*Before you Print* Be sure to set your printer to the following settings in order to print this checklist on one page:

This is a one-page 8½" x 11" checklist
This checklist was created in Word
Make sure your printer settings are set to Portrait
Choose print 1 page only
If the margins of any sections are outside the printable area of the page - click "OK."
