Blood Pressure & Pulse Rate Checklist
This checklist will help you keep track of your blood pressure and pulse, whether you take it at home or have it taken in your doctor's office. You can record your pulse rate and both your systolic and diastolic pressures, up to three times per day. The systolic and diastolic pressure numbers provide important information about your health. The more difficult it is for your blood to flow through your blood vessels, the higher these numbers will be. See the chart below.
